Physical Therapy

Our physical therapists work tirelessly by assessing and evaluating therapeutic, rehabilitative, and functional status of the patient and participate in developing comprehensive care plans. Our goal is to utilize scientific methodologies in order provide thorough care and achieve optimal results.  This is done in part by educating our patients and informing them through each stage of treatment the most effective prevention methods to maximize function during treatment and preventing limitations following treatment.

Your physical therapy and rehabilitation will be based on your unique situation and needs and will be treated by a highly qualified physical therapist who is dedicated to your long-term health and wellbeing.  These practices include but are not limited to:

  • Evaluating the home environment of the patient
  • Initiating PT program and instructs other personnel and/or family/caregiver members in certain phases of PT
  • Identifying patient and family/caregiver needs for other home health services and refers as necessary
  • Participating in peer consultation and case conferences

Occupational Therapy

Human Touch Health Care Agency Inc. also offers occupational therapy services in order to help patients of all ages do what they desire through the therapeutic use of everyday activities.  Our occupational therapists enable people of all ages to live a high-quality life by promoting healthy habits and activities in order to prevent-or live better-with a physical disability or illness.

Our occupational therapists possess a comprehensive perspective when it comes to our patient’s needs in order to help patient’s adapt to their environment.  All our occupational therapy practices that are meant to improve patient’s lives are based on evidence-based practices that are deeply-rooted in scientific fact.  These practices include but are not limited to:

  • Evaluating the patient’s functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, and work capacity)
  • Evaluating the home environment for hazards or barrier to more independent living
  • Developing a treatment program and establishing goals for improved function.
  • Attending and contributing to in-services, case conferences, and other meetings to ensure a comprehensive plan of care

Speech Therapy

In addition to occupational and physical therapy services, the Human Touch Home Health Care Agency, Inc. provides quality speech-language pathology services.  Speech disorders can occur when a person is unable to produce speech sounds correctly or fluently or has issues associated with their voice or resonance.  As far as language disorders are concerned, these occur when a person has difficulty understanding others or sharing thoughts, ideas, and feelings.  Our speech language pathologist employ and utilize only the most scientifically proven methodologies in order to maximize positive patient outcomes.  These practices include but are not limited to:

  • Conducting appropriate evaluations, assessing the home environment, and identifying equipment needs relative to speech/language function
  • Identifying functional speech defects and establishing a plan of care to improve a patient’s function
  • Conferring with referring physician and other organization personnel to ensure coordinated and comprehensive care
  • Administering speech therapy program utilizing specialized therapeutic technique and/or equipment; recommending communication devices/aids as indicated
